Personalized care should fit into real life. For members, visits may be provided at home or at work, offering a more convenient and flexible way to receive care in the setting that works best for you. This service is available exclusively to established members and is ideal for select acute concerns, follow-up care, and other clinically appropriate visits. Scheduling is based on location, availability, and medical appropriateness.
Cancellation Policy
Online Booking Policy Appointments scheduled through our website are subject to review and confirmation by the practice. Membership status: Membership benefits begin only after all required forms and agreements have been completed and accepted by the practice. Selecting or paying for a membership plan online does not by itself activate membership. Included member visits: Active members receive one included visit per month as part of their membership. If more than one appointment is scheduled in the same month, additional visit fees may apply. Non-member visits: Patients who are not active members are welcome to book available appointments at the current non-member rates. Cancellations and rescheduling: If you need to cancel or reschedule, please do so at least [24 hours’ notice] in advance whenever possible. Late cancellations or missed appointments may be subject to a fee. No-shows: Missed appointments without notice may result in a no-show charge and may affect future online booking privileges. Appointment approval: All online appointment requests are subject to availability, eligibility, and clinical appropriateness. The practice reserves the right to adjust appointment type, reschedule, or decline an appointment request when needed.
